Doing it from a fresh boot into the DOS utility is the safest way to do it.some products, like Acronis TrueImage will actually do the backup from within Windows while Windows is running.but that seems like voodoo to me and I don't trust it.
It will reboot you into the DOS utility and automate it for you, so you don't have to do anything. And with the 2003 and later versions, you can do all functions from Windows.
Ghost is the most user friendly out there.
